Terms and Conditions:

1. Valid 30 days from the date of your first weigh in and
    only for weight loss of less than 6 lbs over the
    30 day period. *Exception may be made if you
    are on medication known to slow weight loss or
    if you have less than 30lbs to lose. SureSlim
    BMI chart is the standard to be used to determine
    ideal weight.

2.You must weigh in at the SureSlim clinic weekly
   and participate in the support program.

3. You must fill out a weekly Menu Planner, with
    all necessary information, and share this with
    your consultant.

4. You must follow the program and all advice from your consultant,
    (which will be recorded), without deviation.

5. SureSlim scale weights will be the only valid scale weight.

6. In the event a refund is agreed to, you must return all hard copy and other information supplied
    and verify, by signature, that no copies of the SureSlim material have been retained, shared or
    distributed in any way whatsoever.

7. Your refund excludes the cost of blood tests, doctor fees, banking charges
     and $150 handling charge.

8. Refunds will be issued 60 days after you start the SureSlim program.

9. Above terms and conditions are over and above the terms on the back of the SureSlim
    enrolment form. Some Provinces may also have other terms which may also apply.
